The Alaska Bureau of Vital Statistics now provides
the application for a Medical Marijuana Registry card through
its web site. Choose the marijuana packet link above to obtain
the necessary forms.
These forms are PDF documents requiring Adobe Acrobat
Reader. If you do not have Adobe Acrobat Reader installed on your
computer, please click on the Adobe logo below to download the
free software now.
Important: Please read the instructions and statutes
included in the packet carefully. Failure to do so could result
in denial of your marijuana registry card. If you are denied,
you will not be allowed to reapply for a period of at least 6
months (AS 17.37.010(i)).
