relatives may purchase an Heirloom Birth gift certificate
to be redeemed by an individual or parents of a child born in
Alaska. Certificates may only be purchased for births which
occurred in Alaska.
- ALL REQUESTS MUST INCLUDE A COPY

it is clear and readable when sent to the Bureau. A signature
under the copied ID is also required.

Vital records requests contain confidential information. Therefore,

convenient, is not secure and subject to fraud. Be sure to sign
your request and enclose the correct fees and a copy of picture
ID. If faxing, the Alaska Bureau of Vital Statistics will call
to confirm receipt within 1 business day. If you do not receive
confirmation, call (907) 465-3391 Monday-Friday, 8 a.m. - 5
p.m. Alaska time.
- PROCESSING TIME - Allow 3-4 weeks for the processing

allow for additional mailing time.

Trust and the Alaska Bureau of Vital Statistics, Heirloom Certificates
