The Alaska Bureau of Vital Statistics now provides
cultural adoption paperwork through it's web site. Choose the
adoption packet link above to obtain the necessary forms.
These forms are PDF documents requiring Adobe Acrobat
Reader. If you do not have Adobe Acrobat Reader installed on your
computer, please click on the Adobe logo below to download the
free software now.
Please note that Affidavit of Paternity forms
are not provided in the adoption packet and must be obtained directly
from the Bureau. To request this form, or for additional information
regarding cultural adoptions, please call (907) 465-8601.
